Engineers and Fabricators Company Docket No. 99900320/78-01 NOTICE OF DEVIATION Based on the results of an NaC inspection conducted on December 4-7, 1978, it appeared that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:

A.

Criterion V of Appendix B ta 10 CFR 50 states in part that " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with there instructions, procedures, or drawings...."

In addition, Paragraph 2.1.3,Section IX of the ASME accepted QA Manual states in part that " The Departmental Process Sheet entry for each activity controlled by a procedure includes the number and revision as needed to identify the procedure to be followed.

These activities shall be performed in the specified sequence and no activity shall be started until the prerequisites have been satisfactorily completed."

Contrary to the above, component fabrication in the shop was not being performed in the specified sequences.

Specifically cited are examples identified as Departmental Process Sheets for job Numbers 18285, 18386, 18384, and 18383.

B.

Criterion XII of Appendix B to 10 CFR 50 states that " Measures shall be established to assure that tools, gages, instruments, and other measuring and testing devices used in activities affecting quality are properly controlled, calibrated, and adjusted at specified periods to maintain accuracy within necessary limits."

In addition, paragraph 7.1.1 of Procedure IP-XII-02 requires the Quality Control Manager to prepare a master list of equipment / tools including name, model/ serial number, sarvice schecule, and location of maintanence/ operating manuals.

Contrary to the above, a master list of equipment / tools has never been prepared.
