Appendix A NOTICE OF VIOLATION Union Electric Company Docket No. 50-483 Based on the inspection conducted on April 23-24, 1980, it appears that certain of your activities were in noncompliance with hTC requirements, as noted below. These items are infractions.

1.

10 CFR 50, Appendix B, Criterion V, states, in part, that " Activities affecting quality shall be prescribed by documented instructions, pro-cedures.

. and shall be accomplished in accordance with these instructions, procedures, or drawings."

SNUPPS Union Electric Company Quality Assurance Manual Section No. 17, i

Table 17.1-1, states, in part, that " Safety-related construction acti-vities shall be conducted in accordance with documented procedures drawings, instructions, checklists, or their equivalent.

Contrary to the above, several of the surface mounted component sup-port plates were not installed in accordance with design drawing requirements in that the concrete expansion anchor bolts and weld attachments were mislocated. Furthermore, the deficiencies were not identified and reported to design engineering for resolution.

10 CFR 50, Appendix B, Criterion X, states, in part, that "A program for inspection of activities affecting quality shall be established 1

and executed by or for the organization performing the activity to verify conformance with the documented instructions, procedures, and drawings for accomplisliing the activities."

SNUPPS Union Electric Company Quality Assurance Manual Section No. 17, l

Table 17.1-1, states, in part, that " Inspection of safety-related

construction activities shall be performed to verify conformance with j

documented instructions, procedures, and drawings," and " Control measures shall be established to identify the need for corrective action, to provide for determination of the cause, and appropriate corrective action to prevent recurrence."

Contrary to the above, none of the installed hangers and snubbers had been QC inspected to verify piping suspension system location, con-figuration, and condition. The QC inspection program did not require timely inspection to ensure prompt identification of nonconformances, initiation of corrective action, and program evaluation to prevent recurrences.
