Description of Event

A periodic test, 1-PT-57.7, whose purpose is to check for leaks in the Low Head Safety Injection System was not performed within its 92 day surveillance interval. The test became effective 12/17/79 and was not performed until 7/22/80.

Probable Consequences of Occurrence The consequences of not performing the test were minimal since only two minor leakages were identified. Since monthly periodic tests were performed to verify operability of the LHSI system, the health and safety of the public were not affected.

Cause of Event

The Operations Department was not made aware of the existence of this test which became effective 12/17/79.

Immediate Corrective Action

The test was performed on 7/22/80 and completed satisfactorily.

Two minor discrepancies were discovered: a flange leak on FE-1948, a body to bonnet leak on 1-SI-307. Maintenance reports were issued to repair the above items.

Scheduled Corrective Action No scheduled corrective action is required.

Actions Taken to Prevent Recurrence The Performance Engineer has altered the logging process so as to facilitate timely notification to the appropriate department of any new periodic test that is issued.
