EEVT DESCRIPTION Duri g normal operation a routine safety injection tank makeup was performed according to approved plant procedures.

The supervisor completing the procedure contacted higher supervision indicating he could not meet the acceptance criteria of the procedure and questioned the reason for the upper limit on Boron concentration.

The supervisor was informed that the acceptance criteria upper limit was based on the new accident analysis limit for boron precipitation.

Even through the safety injection tank boron concentration was' exactly at the safety analysis limit and greater than the conservative acceptance criteria of the procedure the concentration could not be decreased.

due to time limitations on the length of time the safety injection tank recirculation valve may be open during the month imposed by Technical Specification Change No. 126.

A subsequent review of procedures completed since start up of Core XII revealed that on ten (10) occasions the acceptance criteria had been exceeded during makeup without any corrective action and three (3) of these exceeded the safety analysis limit of 2400 ppm.

This incident is reported as Reportable Occurrence 50-29/76-06.

CAUSE DESCRIFFION Change 117, Supplement 3, Enclosure A required specific r.pper limits,

on boron concentration in the safety injection tank, safety injection accumulator and Main Coolant system to preclude boron precipitation in the unlikely event of a loss of coolant accident.

The potential consequences from this incident have been found to be minimal since a calculation of total ppm of boron based on volume weighed average conc.entration was less than that assumed in the accident analysis.

Previous cores had not required an upper limit on boron concentration.

Though the makeup procedure had been revised consistant with the safety analysis, the procedure had been performed with apparent disregard for the acceptance criteria.

Consequently, the boron concentration linits were exceeded in an attempt to leave a conservative margin to the low limit.

Chemistry calculations for makeup did not consider a high limit for similar reasons.

The personnel involved were reminded of their responsibilities to l

strictly adhere the plant procedure requirements.
