EVENT DESCRIPTION

During the surveillance testing the Iodine Channel of the Primary Vent Stack Radiation Monitoring System failed to meet the count rate acceptance criteria.

I G C Supervision was notified and an inves-tigation initiated. The source was found to be out of alignment and did not stop in front of the detector. A means for manual sampling and counting for iodine was still availabic.

There is no record of a previous malfunction of this nature.

The test source was realigned.
