fPPENDIX A NOTICE OF VIOLATION Tennessee Vallev authority License No. DPR-77 Sequoyah Unit 1 Based on the NRC inspection September 16-19, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.

As required by Technical Specification 6.9.1.11, Reportable Occurrences l

shall be reported to the NRC in accordance with the time requirements' set forth in Technical Specifications 6.9.1.12 (24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> prompt notification" with 14 day written followup) and 6.9.1.13 (thirty day written report).

Additionally, Technical Specification 4.8.1.1.4 requires that all diesel generator failures shall be reported and that the report shall include the information recommended in Regulatory Position C.3.b of Regulatory Guide 1.108, Revision 1, August 1977.

Contrary to the above, in the time period May 1 through September 8, 1980 three 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> written reports, one 14 day report, and twenty-two 30 day reports were submitted late. Additionally, in the same time period, ten diesel generator failure reports were submitted that did not contain all of the information required by Technical Specification 4.8.1.1.4.

This is an infraction.
