Dear Mr. Engelken:

In accordance with Technical Specifications Section 6.9.4.1(i) and Regulatory Guide 1.16, Revision 4, Section C.2.a(9), the Sacramento Municipal Utility District reports the following Licensee Event Report Number 81-8.

During preventative maintenance on ITE Electrical Breakers Model K-600, a breaker was observed not to crip. An investigation revealed that a wire had slipped out of a terminal lug from the tripping coil.

The coil wire was measured to be No. 20 gauge and the mating lug was for 16-14 gauge wire.

A check of other ITE electrical breakers indicated that the problem is generic to these breakers.

Mr. R. Colombo discussed details of this problem with the Resident NRC Inspector, H. Carter.

A complete report will be submitted to your of fice within fourteen days.
