i MAR tt WR MEMORARDUM FOR: Charles E. Rossi, Director Division of Operational Events Assessment, hkR I
Brian K. Grimes, Director Division of Reactor Inspection and Safeguards, NRR FROM:
Thomas M. Novak Director Division of Safety Programs, AEOD
SUBJECT:
FAILURES OF ELECTRICAL SUPPLY AND POWER GEhERAT10h EQUIPMENT WHICH DISRUPTED PLANT FUNCTION AT NUCLEAR p" -
POWER PLANTS
(
Enclosed is the subject engineering analysis. This ttudy was initiated because of the occurrence of several events in early 1989 involving major electrical equipment or offsite power problems. The issues addressed in this re rern disruptions of normal stetdon auxiliary alternating current (AC) port con-power supply caused by grid, switc'a ard, and t on-1E electrical system problems and major electrical equipment failures.
~
Most of the operating experience cited in this report was previously informally provided to W. Johnston, Region 1, several months ago and provided the basis for his work to develop a ternporary inspection module. This report provides a compilation of that data as well as additional operating experience.
I believe that the operating experience enumerated in this report particularly on maintenance, surveillance, and procedural problems, will be of use to NRR in developing procedures for the upcoming electrical team inspections.
Failures addressed in this report include major equipment failures which is addressed in AE0D/E905, " Electrical Bus Bar failures," and in on-going studies on main transformer f ailures and f ailure of the fast bus transfer scheme.
We are forwarding this Engineering Evaluation to you for your information.
If you have any questions or comments, please do not hesitate to contact me or Mary S. Wegner (x27818) of my staff. We shall be happy to meet with you or your staff to discuss this matter, g' igir.s1 M 38 0 #d Thomas M. Novak Dirfetor Division of Safety Programs, AEOD
