ALLEGATION OF FALSIFICATION OF QC RECORDS l

i A former welding inspector at the D. C. Cook 2 facility (currently

--under construction-) has alleged that certain' nondestructive examination

  • records, radiography, and magnetic particle testing have been falsified l

and that QC inspectors had been told to sign off on inspections that i

they knew to be improper. The individual stated that, during his one i

year employment, he had worked on systems in the auxiliary building and j

in containment and can name systems and, in some cases, specific welds i

that he alleges are improper.

i y-The individual hasTalso' contacted the Berrien County, Michigan, Prosecu-t tor's Office because he-felt the death of two workers,--due to argon gas asphyxiation on September 17, 1976, (see PNO-76-158), was partially F

due to an improper quality control inspection.

RIII personnel have made an appointment to neet with the individual on April 7, 1977. The individual is unavailable earlier, due to a move of his residence.

No press release has been made. Nomediainquiryktave'beenreceived.

. Region III was notified at 3:30 p.m. on March 30, 1977, by the Berrien County Prosecuto of the alleger's concerns. The RIII investigator subsequently contacted the alleger by telephone.
