Sandwich Community High School of Sandwich Illinois, hereby applies for t

renewal of its license #3NM-1223 (Docket #70-1271), pursuant to the telephone conversation between you and Mr. James Liptrap, physics teacher.

The following information is submitted in fblfillment of the requirements of Section 70.22 Title 10, Code of Federal Regulations, Part 70 "Special Nuclear Materials." Two copies are being submitted.

1. Sandwich Community High School is a part of Sandwich Community Unit School District Number 430 in DeKalb County in the State of Illinois. The principal office of the school district is located at 720 South Wells Street, Sandwich, Illinois, 60548. The high school is located at 515 East Lions Road, Sandwich, Illinois, 60548.

2. The noutan source is no long ~ in use. It is kept in storage in the neutron howitzer described in our application letter dated November 12, 1970, in a corner of a locked storeroom between two classrooms in the interior of the high school building. The storeroom is kept locked when not in use, and the key to the howitzer i

is stored in the school safe.

3. The license renewal is requested for an additional period of time such as may be needed to find a buyer for the material or to find a manner of disposing of it.
4. The amount of special nuclear material under this license will continue to be the 150 milligram, 2.07 curie Plutonium-Beryllium source obtained October 20, 1971, from Monsanto Research Corporation of Dayton, Ohio, Source Number MRC-Pu8Be-205 8612050455 861020 REG 3 LIC70 SNM-1223 PDR

5. Dr. Kenneth Ehren is no longer employed by the Sandwich School District, and has moved to another state. The physics program is now under the supervision of Mr.

James M. Liptrap, who received the degree of Bachelor of Science in tha Teaching of Chemistry in 1973 from the University of Illinois, and the degree of Master of Education in Secondary Education in 1975 from the same institution. He has no specific training in the handling of radioactive materials, and is therefore not using the regulated source herein described.

6. The howitser used is the same as specified in our original application. The last use of the protection equipment and the last record of exposure was in June of 1978 when Mr. Ehren resigned. The materials have been in storage since then.
7. Mr. Ehren last performed a leak test on May 24, 1978.

Dr. William J. Adam of your office performed a leak teet for us in June, 1980, but left no written record thereof. The source was found not to have leaked.

8. Any theft o: loss of special nuclear material for which this renewal is requested will be reported to the Commission in accordance with section 20.402 of 10 CFR 20 following a telephone call to your office to find out what this section says. Your office will also be notified of our intention to transfer the material either to another educational institution or to a disposal agency so that one of your radiation specialista may monitor the packaging and paperwork involved.
