Dear Mr. Johnson:
On January 13, 1986, we verbally notified your Mr. T. F. Westerman of a deficiency regarding the placement of Richmond inserts in the proximity of loaded embedded plates which could compromise the structural integrity of adjacent shear cones. This is an interim report of a potentially reportable item under the provisions of 10CFR50.55(e). We have submitted interim reports logged TXX-4700 and TXX-4919 dated February 12, 1986, and July 18, 1985, respectively.
Prior to March 3,1986, design and installation requirements did not identify minimum spacing requirements between loaded Richmond inserts and loaded embedded steel plates. As a result, conditions could exist in which the attached item (s) could have overlapping shear concrete cones and exceed the local structural design loading values. Our evaluation has revealed similar conditions exist for the spacing between Hilti bolts and loaded embedded plates. The scope of this evaluation is being expanded to encompass these conditions.
To prevent recurrence, specification 2323-SS-30, " Structural Embedments," has been revised and Construction Procedure Instruction CEI-20 is being revised to clarify concrete anchor spacing criteria.
Construction procedure CCP-45,
" Permanent and Temporary Attachments to Embedded Weld Plates," has been revised to clarify installation criteria and construction attributes. Quality Control Procedure QI-QP-II.14-13, " Separation Verification of Attachments to Loaded Embedded Plates," has been developed to address the inspection of attachments to embedded plates and spacing requirements for welded attachments, loaded Hilti bolts, and Richmond inserts.
This procedure will also address the inspection of previous and in-process installations.

TXX-6031 October 20, 1986 Page 2 Due to the revised scope of this issue, the identification (through field verification and subsequent evaluation) of Unit 1 installations involving the spacing of anchors / attachments is scheduled for completion by January 16, 1987.
In Unit 2, the Wall Embedments and Beams (WEB) program will provide identification and analysis of all embedded plate attachments. A plan for the reinspection of existing Unit 2 embedded plate attachments is in development.
We anticipate submitting our next report no later than February 6,1987.
