Good Afternoon Dr. Wagner, We have a follow up question based on our conversation with you on April 6th regarding the anecdotal data you provided for candidates that received the ABR certification. You mentioned that the ABR certifies about 2,400 individuals per year in all disciplines, and that Diagnostic Radiology represents more than half of this total number and only 10% of DRs go on to be AUs. Does that 10% also apply to other specialty areas that are recognized by NRC (for the AU, AMP, and RSO)?

Good morning David, Thank you for following up with Jenny and for clarifying the question I had regarding the radiation oncology exam content. You mentioned in your May 27th email that the ABR would recommend that the NRC remove the sample certificates from the section you highlighted from our website because the RSO-eligible and AMP-eligible designations issued on these certificates are more than 7 years past training and as a result are no longer valid.
Note that once the certificates are removed from the NRC website, they will no longer be seen as NRC-recognized certificates. The NRC keeps the specialty board recognized certificates that were issued beyond the 7-yr time frame in accordance with 10 CFR 35.59, Recentness of Training for a few reasons:
- 1. There may be individuals who at one time met the board certification requirements and received their certificate with AU/AMP/RSO Eligible, but have not been involved in licensing medical-use activities for extended periods of time (7 years or more). If these individuals end up seeking to become involved in these activities again as AUs, AMPs, or RSOs, they can still use these older recognized certificates along with documentation of continuing education, as long as the certificate is listed on the NRC website.
- 2. For historical knowledge and questions that may arise regarding past certificates.

As we were processing the ABR certificates, we noticed that some of the dates overlapped by 1 day. You do not need to submit new documentation, but I am looking for a confirmation. Regarding the Radiation Oncology exam content as outlined in 10 CFR 35.490(a)(2) and 10 CFR 35.690(a)(2) - we have general references in the three qualifying exam study guides that cover all of the topics listed. However, we think it would be useful to update the study to more direct reference the requirements. Were working on that and should have that done sometime next week.
Regarding the medical physics certificates. I am sorry for the delay in getting these sent. Weve been struggling with an old certificate vendor that we havent worked with in more than ten years. Please find a description of what we have below.
In addition, we would recommend the NRC remove the sample certificates from the highlighted groups below from your website because the RSO-eligible and AMP-eligible designations issued on these certificates are more than 7 years past training and as a result are no longer valid.

- b. Explain how the ABR verifies that all its candidates have advanced degrees in the appropriate fields from an accredited college or university (in accordance with 10 CFR 35.51(a)(1). Note that the NRC requirements in 10 CFR 35.51(a) do not require medical physicists to complete residency programs.
- c. Explain how the ABR verifies that all its candidates have 2 years of full-time practical training and/or supervised experience in medical physics (in accordance with 10 CFR 35.51(a)(2)) - under the supervision of a medical physicist who is certified in medical physics by an NRC or Agreement State recognized specialty board OR by a physician that meets the requirements in 10 CFR 35.57, 35.490, or 35.690. You just stated that the residency programs met CAMPEP accreditation requirements.
Note that all sample certificates (referenced in this email and my email sent on May 9, 2022) should include the correct degree designations, the issue and valid through dates, and the watermark NOT VALID.

You did not explain how the ABR verifies that all its candidates have 2 years of full-time practical training and/or supervised experience in medical physics (in accordance with 10 CFR 35.51(a)(2)) - under the supervision of a medical physicist who is certified in medical physics by an NRC or Agreement State recognized specialty board OR by a physician that meets the requirements in 10 CFR 35.57, 35.490, or 35.690. - 3. Provide corrected sample certificates that include the correct degree designations and issue and valid through dates. Some of the ABR sample certificates for 10 CFR 35.50 and 35.51 do not reflect the necessary degree designations. Some certificates do not have a degree listed at all, and some have an M.D. listed instead of a masters or doctorate degree. Furthermore, some of the issue date ranges and valid through dates may not be correct. Make sure that each replacement sample certificate that you will submit includes the watermark NOT VALID.

In 2011, the ABR changed the name of Radiologic Physics to Medical Physics:
- Therapeutic Medical Physics replaced Therapeutic Radiologic Physics.
- Diagnostic Medical Physics replaced Diagnostic Radiologic Physics.
- Nuclear Medical Physics replaced Medical Nuclear Physics.
The last time-limited certificates issued under the name of Radiologic Physics were valid through 12/31/2020 as long as the diplomate was meeting Maintenance of Certification requirements.

The NRC specialty board webpage, lists a total of four NRC-recognized ABR specialties for the RSO/ARSO (Diagnostic Radiologic Physics, Diagnostic Medical Physics, ML22161A335
Medical Nuclear Physics, and Nuclear Medical Physics) and two for the AMP (Therapeutic Radiologic Physics and Therapeutic Medical Physics).
Please review and confirm the certificates that are still valid for the RSO/ARSO and AMP training. For any certificates that are no longer valid, provide the date that they stopped being issued.
Diagnostic Medical Physics, Nuclear Medical Physics and Therapeutic Medical Physics are all listed (see screenshot below). We do not list Diagnostic Radiologic Physics, Medical Nuclear Physics and Therapeutic Radiologic Physics because time-limited certificates with these names were last issued in 2010 (expired 12/31/2020).
- 3. Item 5 of your letter states that your Diagnostic Medical Physics, Nuclear Medical Physics, and Therapeutic Medical Physics certificates are not current. When can we expect to get the updated certificates?

I also wanted to let you and Dr. Wagner know that today was our NRC Advisory Committee for the Medical Uses of Isotopes (ACMUI) Annual Meeting, and the ABRs intent to terminate NRC recognition of all certification processes came up in a question to NRC, and it was a hot topic of discussion. We also received a lot of comments on this and the potential impacts on the ML22161A335
medical community. This item was not a topic on the meeting agenda, but the meeting information (agenda, presentation slides, and meeting transcripts) can be found here: https://www.nrc.gov/reading-rm/doc-collections/acmui/meetings/2022.html. The transcripts are not yet available on the website. We will share the transcripts from the meeting with ABR once we receive it from our court reporter.

NRC Receipt of the ABR Termination Letter dated March 29, 2022 --- RE: ABR Good Afternoon Dr. Wagner, We have received the American Board of Radiology (ABR) letter dated March 29, 2022, informing the NRC of the ABRs intent to eliminate its processes for offering Authorized User -
Eligibility designation on ABR certificates awarded after December 31, 2023; and delegating you with the responsibility and authority to act as the ABR designee for all official correspondence with the NRC, including this notification letter. Note that your request for termination of specialty board recognition with the NRC is now under review and we will be responding to you in a formal termination letter.
We still plan to respond to your request for recognition of the ABRs Nuclear Radiology certification process, and will follow up in a separate email with the deficiencies identified.
We look forward to your response to our March 15th letter regarding our review of ABRs continued recognition.

Good Afternoon David, Can you please indicate whether or not the ABR letter dated March 29, 2022, informing the NRC about the ABRs plan to terminate NRC recognition of all ABR certification processes contains information that should be withheld from the public in accordance with 10 CFR 2.390(b)? If it is not, please indicate when the ABR plans to make this publicly available.
