Dear Dr. McCrea:
This refers to the inspection conducted by Mr. K. R. Naidu of this office during February 6-10, 1989, of your facilities. located in Neponset, Highland, and Cocassett, Massachusetts and to the discussions of our findings with you and members of your staff at the conclusion of the inspection.
This inspection was performed as a result of inspection findings identified at 'j the Millstone 2 nuclear power plant relative to defective pressure signal limiter' printed circuit boards (PCB) supplied by you. Specifically, the PCBs were inoperable and would not fit into the holders, which were also provided by you. This ins assurance (QA)pectionprogram inalso reviewed selected areas. the implementation of your quality
-Areas examined during this inspection and our findings are discussed in the enclosed report. Within these areas, the inspection consisted of an examina-tion of procedures and representative records, interviews with personnel, and observations by the inspectors.
During this inspection, it was determined tnat the implementation of your QA program failed to meet certain NRC requirements. Specifically, the Nuclear Safety Subcommittee was not convened to evaluate problems identified in repair orders to determine the extent gnd appropriateness of corrective actions necessary to complete the repairs.
