NOTICE OF VIOLATION PECO Energy Docket Nos. 50-352;50-353 Limerick Generating Sthtion Units 1&2 License Nos. NPF-39; NPF-85 During an NRC inspection conducted between June 1,1998, and June 24,1998,a violation of NRC requirements was identified, in accordance with the General Statement of Policy and Procedure for NRC Enforcement Actions, NUREG-1600,the violation is listed below:

A. 10 CFR 50, Appendix B, Criterion XVI, requires, in part, that: " Measures shall be established to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, defective material and equipment and nonconformances are promptly identified and corrected."

Contrary to the above, the following conditions adverse to quality were not promptly identified and corrected.

1. Defective equipment (an incorrectly wired current transformer) in the supply circuit breaker of load center 124B was not identified and corrected, and on March 22,1998 the breaker tripped unexpectedly and de-energized all the loads supplied by the load center. PECO was aware of potential wiring errors affecting the circuit breakers used at Limerick since November 1996 based on industry operating experience. PECO had failed to revise the periodic calibration procedure, which could have identified the incorrectly wired current transformers and prevented the event.
2. Deficiencies (age-related failures) in Agastat relays in three panels (OA, OB, j and OC-C124) were not identified and corrected. Due to higher panel l temperatures, the continued use of these Agastat relays beyond their appropriate service life led to higher age-related failures, including the April 4,1998, event in which the B train of the control room emergency fresh air supply (CREFAS) system did not actuate. On June 19,1998, several such relays remained in service, considerably beyond their calculated service lives.

This is a Severity Level IV Violation (Supplement 1).

required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region I, and a copy to the NRC Resident inspector at the f acility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation or severity level, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Your response may reference or include previous docketed correspondence, if the correspondence adequately addresses the required response. received within the time specified in this Notice, an order or a Demand for Information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time. j If you contest these enforcement actions, you should also provide a copy of your response to the Director, Office of Enforcement, United States Nuclear Regulatory Commission, Washington, DC 20555-0001.
