Gentlemen:

This is in reference to your request dated December 8, 1986 for amendment of

your byproduct material license. In order to complete our review, we need the

following additional information:

1. It was not clear from the information submitted, that the physicians'

training and experience satisfied the requirements in Policy and

Guidance Directive FC85-01, copy enclosed. In particular, the number of

hours of training as described in each group of Enclosure 1 should be

submitted for each physician to be added to the license.

< Also, they should submit the dates of their Lixiscope training from Dr.

Paton.

v Finally, they should submit information on their state licensure to

practice medicine.

/.2 A statement should be submitted from the management of the Alaska Native

Medical Center stating that they wish to add the physicians to the

license.

We will continue the review of your amendment request upon receipt of this

information. In order to continue prompt review of your application, we

request that you submit your response to this letter within 30 calendar days

from the date of this letter. Please reply in duplicate, and refer to Mail

Control No. 70503.
