NOTICE OF VIOLATION (NRC INSPECTION REPORT N0. 50-294/86001(DRSS))

This refers to the routine safety inspection conducted by Mr. R. A. Paul of this office during June 19-26, 1986, of activities at the Michigan State University TRIGA Reactor authorized by NRC Operating License No. R-114. Our letter to you dated July 15, 1986, forwarded a copy of our report of the inspection. As noted in that letter an unresolved item concerning

, discrepancies in your area radiation monitor calibration records was identified during the inspection. Enforcement action pertaining to the unresolved item was postponed pending receipt of additional information concerning your activities.

Upon further review of this matter, including the information provided in your letters to us dated August 5, 1986, September 8, 1986, October 31, 1986, and November 21, 1986, it appears that your activities violated NRC requirements as described in the enclosed notice. With one exception your aforementioned letters have satisfactorily responded to our concerns that your future activities will be performed in compliance with your license requirements and that the identified violation has been corrected. The exception has been resolved by your staff's verbal commitment to modify the area monitor calibration procedure to include an acceptance range for future calibrations.

Consequently, no reply to the violation is required and we have no further questions regarding this matter at this time. During a future inspection, we will review your implementation of the corrective acticns you have taken to ensure a proper instrument calibration program.

In addition to the violation, however, we are concerned that your management oversight did not identify the calibration discrepancies, that your initial written responses to this office were not responsive to our concerns, and that your procedural program was not rigorous enough to allow you or us to determine the calibration procedures in use for the 1981-1984 calibrations. Such performance is below the standards we expect from NRC licensees. Your letters and discussions with members of your staff lead us to believe that you will institute appropriate corrective actions for these weaknesses. We intend to review these and other management control areas further during a future inspection in order to better evaluate the effectiveness of your corrective actions.
