MONTHLY NARRATIVE REPORT OF OPERATION AND MAJOR MAINTENANCE EXPERIENCE This report describes the operating and taajor maintenance experience for-tie month of December,1986. This narrative report was prepared by the plant staff and is submitted in accordance with Section 6.9.1.7 of the Units'1 and 2 Technical Specifications.

On December 1, 1986 Unit I started the month shutdown in its first refueling outage. Unit 2 started the month at full power.

On December 3, 1986 Unit 2 reduced power to 50% to search for leaking condenser tubes.

On December 4, 1986 Unit 2 returned to full power.

On December 5, 1986 Unit 2 reduced power to 50% to search for leaking condenser tubes.

On December 6, 1986 Unit 2 returned to full power.

On December 9, 1986 Unit 1 entered Mode 4.

@ December 11, 1986 Unit 1 entered Mode 3.

On December 12, 1986 Unit 2 reduced power to 50% to search for leaking condenser tubes.

On December 13, 1986 Unit 2 returned to full power after plugging leaking condenser tubes.

On December 20, 1986 Unit 1 entered Mode 2.

On December 27, 1986 Unit 1 entered Mode 1.

On December 28, 1986 During the Unit 1 startup a reactor trip resulted i from a steam flow /feedwater flow mismatch l

coincident with a low steam generator level.

While stabilizing the plant, Unit 1 experienced a second reactor trip due to low-low steam generator level.

On December 29, 1986 Unit I synchronized to the PGandE system. Unit 2 reduced power to 50% to search for leaking l condenser tubes and later returned to 85% power l

which was the maximum power level that could be achieved without incurring tube leakage.

On December 31, 1986 Unit 1 ended the month at 30% while performing startup tests. Unit 2 ended the month with power at 85%.

Unit I completed its first refueling outage which started August 29. On December 28, while disconnected from the PGandE system and at apprcximately 7% reactor power for turbine balancing, Unit 1 experienced a reactor trip.

During return to Mode 3 the turbine generator governor valves were closed.

Closing of the turbine generator governor valves lead to an increase in steam generator pressure. The increase in steam generator pressure caused one steam generator level to decrease to the low steam generator level trip setpoint. Also at this time, the 10 percent steam generator dump valves opened intermittently due to increasing steam generator pressure. The resulting change in steam flow through the steam dump system resulted in a momentary steam flow / feed flow mismatch signal. The low steam generator level in coincidence with the steam flow / feed flow mismatch caused a reactor trip.

While stabilizing the plant after the reactor trip, the reactor trip breakers were closed to allow latching of the turbine generator. Latching of the barbine generator allows the turbine generator stop pilot valves to close. Closing of the piloi valves therefore reduces the plant cooldown rate. During plant stabilization a low-low steam generator level reactor trip signal occurred, opening the reactor trip breakers. The Unit was not critical during the second reactor trip.

Unit I was synchronized to the PGandE system on December 29.

The Unit 1 monthly availability factor was 5.8% and the capacity factor was 0.0%.

Unit 2 operated this month with a unit availability factor of 100% and a unit capacity factor of 93.8%. Unit 2 reduced power 4 times this month. I time to search and plug leaking tubes and 3 additional times to search for leaking tubes.

No challenges to the steam generator safety valves or pressurizer power operated relief valves have been made. Significant Unit 1 safety related maintenance consisted of snubber washer inspections and the replacement of detectors NE-51 and N-42. Unit 2 safety related maintenance consisted of a seal replacement on centrifugal charging pump 2-2.
