ANNUAL REPORT FROM THE UNIVERSITY OF WASHINGTON Facility License _R-73 Docket No. 50-139 A. A Narrative Summarv 21 Doerating Exoerlence inn 1986 The University of Washington Nucisar Reactor was operated for 229.8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> (basel ine to shutdown) and generated 12647 KWH of energy during 1986. The 191 experiment runs consisted of sample Irradiations for neutron activation analysis, Isotope production, class demonstrations, TLD trradiations, radiation damage studies, and callbrations.

B. Unscheduled Shut Downs Date Power Level Cause Feb 14 10 watts No Indication, except rods dropped and dump va l ve opened. Probable cause is the photo diode isolation in the saf ety amp i l fler dropping below operational level as a result of 110 AC noise.

Apr 1 1.5 watts LogN & period trip.

Apr 1 97 Kw LogN & period trip.

Apr 11 95 Kw Same as Feb 14, 1986.

May 1 1 watt LogN & parlod trip.

May 13 95 Kw LogN & period trip.

Just prior to Log N and period trip on May 13, 1986 the Log N gave an erratic output. The prob lem was traced to a f au lty 15 volt de modular power supply in the Log N Instrument that was replaced.

1. Routine calibrations and maintenance as scheduled.
2. On April 3, 1986 performed an extra quarterly maintenance on Linear N recorder. During an operation on April 1, 1986 l the pen movement had f ailed to fol low the Linear N amp i l f ler output. This resulted in the auto-control l er not responding to a slow power increase. The reactor power than exceeded 100 KW for approximately two minutes before corrective action was taken by the reactor operator. The l

maximum power l evel was estimated to be 112 KW f rom the Log N recorder. Region V office of the NRC was contacted on April 2, 1986 concerning this event.

3. M a y 15,19 86: Used a 15 voit de power supp l y to provide the i 15 vol ts needed f or the LogN & period amp i I f ier. On May 23, 1986 Insta l l ed Pol yton Dev ices, Inc., Model No. P33-1 586, i 15 v o l t, i 100 ma de power supp l y in LogN & period amplifier.
4. J une 2, 1986: Rep l aced de voltage power suppl y that suppl ies compensating bias for compensated Ion chambers. The power suppl y that was removed f rom service had an intermintent ac noise superimposed on the de output.
5. The position Indicating potentiometer on the regulating rod was replaced on August 28, 1986. The rep l acement is the same make and model as that removed (Beckman Model 5611 R1k/10k L .5).
6. The contacts f rom the pen carriage to the s i ide wires on the Linear N recorder were replaced on Oct 6, 1986.

i F. Environmental Survevs Outside ihg Facility l TLD monitoring data was col lected outside the reactor buil ding j at locations shown on the enclosed drawing. Results are l tabulated on attached table.

G. Significant Exoosures (over EQA mrem inn vear) l For the year 1986 there were no significant exposures, i.e.,

over 500 mrem for the year.(January through November: December data not ava il ab l e at report time. No incidents occurred in December which would lead to excess personnel exposure.)

The dosimeters each contain two TLD chips that are supplied by R. S. Landauer,Jr & Co, G l enwood, Illinois 60425.

available at the time the report was prepared.
