This is an application for termination of Muskingum College's Special Nuclear Materials License, number SNM-1148. Form 314 and a copy of the Relinquishment of Ownership form for the only source covered by this license are enclosed.

The PuBe source was shipped to Los Alamos on 5-18-98 by my colleague, Dr. John Baxter, while I was out of town. On 10-9-98 I inspected the room and container in which the source had been stored. I surveyed the area with our Ludlum survey meter and found no detectable contamination. ,
