SUBJECT: INSPECTION REPORT N05. 50-327/88-13 AND 50-328/88-13 - INTEGRATED

DESIGN INSPECTION FOLLOW-UP

This letter forwards the results of an inspection of the corrective actions

performed by TVA in response to the findings of the Integrated Design

Inspection (IDI) conducted in TVA's Knoxville offices between February 1 and

February 19, 1988. During this inspection, the team reviewed both the specific

and generic corrective actions associated with the IDI findings that remained

open following our previous inspection that was documented in NRC Inspection

, Report 50-327/87-74 and 50-328/87-74 It should be noted that although all

the IDI findings are indicated as closed, several findings involved licensing

issues which must be resolved by TVA with NRC's Office of Special Projects.

These items then are no longer considered IDI issue's and are closed only for

the purpose of this report. IDI findings requiring resolution by the Office

of Special Projects are so indicated in the issue-specific sumaries contained

in Appendix A to the enclosed report. In addition, for many of the IDI findings

a substantial portion of the corrective actions has been deferred by TVA until

after the restart of Sequoyah Unit 2. In each such instance, the NRC has

reviewed and agrees with the TVA post-restart decision for completion of

corrective action. Such IDI findings were closed by the team by verifying

that the necessary post-restart corrective actions have been entered on TVA's

Corporate Comitment Tracking System (CCTS). The CCTS is used by TVA to track

regulatory comitments and is being relied on by the NRC to provide an

auditable trail to assure that the corrective actions are completed in a,

timely manner.

During the IDI exit meeting on September 11, 1987, the NRC made several general

observations that were programmatic in nature. Thes'e related to: (1) lack of

timely implementation of changes to operating procedures resulting from design

changes; (2) lack of timely corrective action; (3) lack of system integration

and (1) weakness in the design verification process.

The NRC considers that TVA has taken responsible corrective actions to

alleviate the above programatic concerns. However, it is not possible to

assess the effectiveness of these procedural and organizational changes at

this time. The impact of these changes depends largely on their diligent

implementation by TVA and can only be assessed after they have been implemented

for a sufficient period of time. The NRC recomends that the TVA management

periodically evaluate the effectiveness of their corrective actions to the

NRC's programatic concerns to assure themselves that the corrective actions

are being properly implemented and more importantly, are producing the desired

end results. The NRC also recomends that TVA periodically review their CCTS

to assure that the committed corrective actions are implemented in a timely

manner.

In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosure

will be placed in the NRC Public Document Room.
