Dear Mr. Rosen:

Notice is hereby provided that your Qualit/ Assurance (QA) Program Approval for Radioactive Material Packages, Approval No. 0888, expired on December 31,1998.

If you are an NRC licensee and conduct activities under the General Licenses of Subpart C of 10 CFR Part 71, or an Agreement State Licensee subject to the requirements of 10 CFR Part 71, as required by 10 CFR Q 150.20, a QA Program approved by the Commission as satisfying the provisions of Subpart H of 10 CFR Part 71 is required. Since you no longer have a vand QA Program Approval, the General Licenses of Subpart C of 10 CFR Part 71 and registration under the provisions of 10 CFR @ 71.12, to transport or deliver licensed material to a carrier for transport are terminated.

Please note that records required by 10 CFR @ 71.91 and 71.135 are to be retained for a period of three years.
