1 10. Licensed material may be used at the licensee's office area at 4415 North Star

3 Street; Barrow, Alaska, and at temporary job sites of the licensee anywhere in

j the United States where the U. S. Nuclear Regulatory Comission maintains

jurisdiction for regulating the use of licensed material.  ;

1 11. Licensed material shall be used by, or under the supervision and in the physical

1 presence of Leonard F. Helson, Wade R. Hansen, Steven F. Hanser, or other

individuals who have ccmpleted the Troxler Electronic Laboratories, Inc. or

j3 i

Campbell Pacific Nuclear training courses in the use of the gauges and have been '

l1j approved by Leonard F. Nelson, Radiation Safety Officer. The license shall

1 maintain records of individuals designated as users.  ;

leakage and/or contamination at intervals not to exceed 6 months. Any p

The licensee shall conduct a physical inventory every six (6) months to account

for all sources and/or devices received and possessed under the license,
