PRECURSOR DESCRIPTION AND ANALYSIS LER No.: 369/85-004 Event

Description:

Reactor Trip With Stuck-Open SG Relief Valve Date of Event: January 28, 1985 Plant: McGuire 1 EVENT DESCRIPTION Sequence A reactor trip occurred on lo-b SC level when MFW pump tripped on a false low-suction pressure signal. Following the pump trip, a turbine runback did not occur on demand because of a control system failure, resulting in a drop in the SC levels. A manual turbine runback was attempted, unsuccessfully. Low SG level occurred within 1 min, causing RPS trip. AEW automatically initiated.

The SC "B" PORV opened because of instrument drift, remained open for ~-4 min, and caused excessive cooldown and loss of SC inventory. Th e SC PORV was fully isolated by the operators (closing the block valve) about 2 min after it had closed by itself.

The discharge check valve on the' turbine-driven AFW pump failed to close as the pump was secured after following the reactor trip. SG backf low occurred but was stopped when the SC isolation valve was closed. This isolation valve later failed to open' on demand during realignment of the system. The valve was manually opened. Two charging pumps were used to recover vessel level.

Corrective Action The MFW suction pressure transmitter was repaired and recalibrated.

During the 1985 refueling outage, the instrumentation will be modified so that a recurrence of such a single-component failure will initiate a multiple logic trip. The main steam pressure transmitters that activate the SC PORVs will be placed on an active preventative maintenance schedule to help detect calibration drift. Actions are being reviewed to correct the AFW check valve problem and to eliminate the diode failures that caused the turbine-runback failure.
