1 MODiflCATION TO THE STATENENT OF WORK FOR FIN L1647 2.5 Task 5 - Collect and Analyze Mixed Waste Data insert after last paragraph:

Participation by NRC and Agreement State licensees in the survey outlined in Phase !! is considered to be voluntary in nature. As such, the performing organization may only encourage licensee participation. The performing organization may not claim, or make representations, that response by licensees to requests for information by the performing organization is mandatory or required.- Participants will be offered the greatest degree of confidentiality possible. NRC and EPA are not collecting this data with the intent of taking any enforcement actions against survey res pondents.

Unless a future development, involving the protection of the pu)lic health and safety and the environment, warrants the release of the names of the survey respondents, the performing organization will not provide NRC or EPA any information on the identity of any licensees contacted in relation to this project. Except as outlined above, data and results from the survey will only be provided to NRC, EPA or other groups or individuals, as approved in writing by the NHSS PM, in an aggregated format, stripped of any specific licensee identifiers.

2.8 Task 8 - Document Results Insert after last paragraph:

At the conclusion af Task 8 tne performing organization shall retain all raw data that contains information or licensee identifiers (e.g., completed survey forms, follow-up call notes and records, records of interviews with licensees, etc.)regardlessoftheformoftherecord(e.g.,hardcopy,computerdisk, etc.) for seven years. After seven years from tue completion of Task 8 the performing organization shall destroy this information. The performing organization may retain only that information and data for which written approval has been obtained from the HMSS PM.
