This refers- to your letter dated November 26, 198M, for renewal of License Number. SNM-608.- Additional information is needed in' support of your request.
.By - telephone on January 31,,1985', this information was requested; however, our-office has not received a reply to the request as of the date of -this letter.
'As a reminder, we need the following information:
- 1. bnder Item 11, Calibration of Instruments on your application, the information was fine for initial calibration of your new equipment; however, we.need 'to .know the. frequency these instruments will be calibrated and the name and address of the service company that will routinely calibrate or your. procedures and standards-if you plan to calibrate them.
- 2. For personnel ranitoring, if you use .the pocket dosimeters for your primary, legal personnel exposure record, then you need to submit your provisions for proper maintenance and calibration of these dosimeters. If you feel that personnel ' monitoring is not required in accordance with 10 CFR 20, ,
20.202(a)(1), please submit an evaluation of the expected exposures from the- ~
proposed uses and demonstrate that no individual is likely to receive a dose Jin any calendar quarter in excess 'of ' 0.31 rems ' to the whole- body, blood-forming organs, lens of the eye, or~ gonads; 4.69 rems to' extremities; or 1.88 rems to the _ skin from all sources and types. of occupational exposures.
