e PALO YERDE NUcfRAR GENERATING STATION UNIT 1 SPECIAL REPORT 1-SR-86-011 VALID DIESEL GENERATOR FAILIRE DUE TO FAILURE TO MEET START UP TIMES Docket No. STN 50-528 License No. NPF-41 This Special Report is required by Technical Specifications 4.8.1.1.3 and 6.9.2 for a valid Diesel Generator Failure . his report is written in accordance with Regulatory Position C.3.b of Regulatory Guide 1.108, Revision 1, August 1977.

During the performance of the Emergency Diesel Generator "A" Start and Load surveillance test (41ST-lDG01) at 0235 on March 6, 1986, with Palo Verde Unit 1 at 100% power, Diesel Generator "A" did not start within 10 seconds as required by Technical Specification 4.8.1.1.2.a. Diesel Gene rator "A" was shut do m and declared inoperable at 0308 on March 6,1986 and the appropriate Technical Specification ACTION statement was entered.

During troubleshooting, it was determined that the Diesel Generator governor (EGB-50-PLS) output shaft had seized. Re output shaft is connected to the Diesel Generator's fuel control system. The governor regulates the Diesel Generat ar's engine speed by controlling the fuel control system through the positioning of the output shaft. Five nonvalid failures were experienced during troubleshooting starts of Diesel Generator "A." he governor was l replaced and the original governor was sent to the manufacturer, Woodward l Governor Company (W290), for evaluation.

Since Diesel Generator "A" did not s tart , accelerate, and assume its design-rated speed during a valid test, this is considered a valid failure.

This was the third valid failure in the last 100 valid tests for Palo Verde Unit 1 Diesel Generators. A test interval of not more than 7 days is being implemented in accordance with the schedule of Regulatory Position C.2.d of Regulatory Guide 1.108, Revision 1, August 1977.

At 1857 on March 7,1986, Palo Verde Unit 1 was shut down for scheduled outage 4 and maintenance activities. Final adjustments to the new governor were made during a troubleshooting startup at 1130 on March 8,1986. Diesel Generator "A" was unavailable for 2 days, 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> and 22 minutes due to the governor I malfunction. When final adjustments to the governor were completed, scheduled 18 month maintenance began. A valid test was initiated at 0856 on March 21, 1986 and Diesel Generator "A" was declared operable at 1733 on March 21, 1986. Diesel Generator "A" was unavailable for 15 days , 14 hours1.62037e-4 days <br />0.00389 hours <br />2.314815e-5 weeks <br />5.327e-6 months <br /> and 25 minutes due to the governor malfunction and scheduled 18 month maintenance.
