ISSUANCE OF FINAL SIGNIFICANCE DETERMINATION AND NOTICE OF VIOLATION This is to inform the Commission that a White Significance Determination Process (SDP) finding and associated Notice of Violation will be issued on or around September 30, 2002, to American Electric Power Company as a result of inspections at the D. C. Cook Nuclear Power Plant. This finding was discussed in COMEXM-01-001 and was preliminarily characterized as a Yellow finding in an inspection report. After considering additional data provided by the licensee in a detailed analysis, the significance for the finding was determined to be White, a finding with low to moderate importance to safety, which may require additional NRC inspections. This finding involved a failed essential service water (ESW) strainer basket which permitted debris to bypass the strainer and enter the ESW system, resulting in the debris intrusion event experienced at the D.C. Cook Nuclear Power Plant on August 29, 2001. The strainer failed as a result of inadequate installation instructions and maintenance practices.

The inadequate maintenance instruction for the installation of the ESW strainer basket was identified as a violation of 10CFR50, Appendix B, Criteria V, Instructions, Procedures, and Drawings.

The characterization of this finding as White, in conjunction with the one existing White finding associated with the Unit 2 Turbine-Driven Auxiliary Feedwater Pump failure to start, has resulted in two White inputs to the Mitigating Systems Cornerstone in the Reactor Safety Strategic Performance Area. Consequently, D.C. Cook performance is in the Degraded Cornerstone Column of the NRC Action Matrix, as described in NRC Manual Chapter 0305, Operating Reactor Assessment Program.

It should be noted that the licensee has not been specifically informed of this action. The schedule of issuance and notification are:
