The American Society of Mechanical Engineers (ASME) Boiler and Pressure Vessel Code, Rules for lnservice Inspection of Nuclear Power Plant Components,Section XI, 2001 Edition through the 2003 Addenda, as modified by 10 CFR 50.55a, is the code of record for the DAEC Fourth Ten-Year lnservice Inspection Interval.
Due to the configuration of the Reactor Vessel and RHR Heat Exchanger, it is impractical to meet the examination coverage requirements of the ASME Code,Section XI, 2001 Edition through the 2003 Addenda, as clarified by Code Case N-460 . Relief is requested in accordance with 10 CFR 50 .55a(g)(5)(iii). This relief, NDE-R017, is provided in the Enclosure to this letter.
NDE-R017 is requested on the basis of lnservice Inspection impracticality.
NextEra Energy Duane Arnold has determined that the examinations addressed in NDE-R017 were performed to the extent possible. As described in the Enclosure, the use of the proposed alternative examinations for this relief provides assurance of an acceptable level of quality and safety.

Class 2 RHR Heat Exchanger Nozzle to Shell weld.
2. Applicable Code Edition and Addenda
The Code of record for Duane Arnold Energy Center Unit 1 is the 2001 Edition through the 2003 Addenda of ASME Boiler and Pressure Vessel Code,Section XI, "Rules for lnservice Inspection of Nuclear Power Plant Components" as modified by 10CFR50, Section 50.55a.
The welds were examined with the latest techniques, procedures, equipment, and personnel qualified to the requirements of the Performance Demonstration Initiative (POI) Program, in accordance with the requirements of the ASME Code Section XI, 2001 Edition through the 2003 Addenda.
3. Applicable Code Requirement

Essentially 100% volumetric examination of the nozzle-to-shell C-B C2.21 weld NextEra Energy has invoked ASME Section XI Code Case N-460, which states: "when the entire examination volume or area cannot be examined due to interference by another component or part geometry, a reduction in examination coverage on any Class 1 or Class 2 weld may be

Therefore, NextEra Energy is requesting relief only for those welds that were inspected and received Code coverage of 90% or less.
- 4. Impracticality of Compliance Due to the configuration of the Reactor Vessel and RHR Heat Exchanger, it is impractical to meet the examination coverage requirements of the ASME Code,Section XI, 2001 Edition through the 2003 Addenda, as clarified by Code Case N-460. Relief is requested in accordance with 10 CFR50 Section 50.55a(g)(5)(iii). These areas of examination limitations were identified during the 4th lnservice Inspection Interval.
When examined, the welds and component listed within this request did not receive the required code volume coverage due to their configuration and/or the presence of permanent attachments.
These scanning limitations prohibit essentially 100% ultrasonic examination coverage of the required examination volume; however all of the subject welds were examined to the extent possible.
Details of the examination limitations, including diagrams depicting the limitations, are as follows:
RPV Course 1 Vertical Welds (VLA-A001 and VLA-A002)
Examination Category 8-A, Item 81 .12 The mechanized scanning device used for the examination of the Course 1 Vertical Welds encountered physical obstructions from permanent component integral attachments and instrumentation supports as follows: Access to weld VLA-A001 was physically obstructed by the jet pump restraint bracket at elevation 229.5", the jet pump sensing brackets at elevations 161.5"
& 153.5" the jet pump sensing line brackets at elevation 112.25" and the jet pump support plate between the O degree and 90 degree azimuths. Access to weld VLA-A002 was physically obstructed by the jet pump restraint bracket at elevation 229.5", the jet pump sensing brackets at elevations 163.25" & 152.0" the jet pump sensing line brackets at elevation 102.5" and the jet pump support plate at approximately azimuth 270 degrees. Figures 1 through 10 provide detailed description and illustrations of the weld volume limitations due to jet pump restraint brackets, the jet pump sensing line brackets and the jet pump support plate.
The examination volume requirement depicted per Figure IWB-2500-2 volume is limited due to the proximity of the shroud support, Jet Pump restraint brackets and Jet Pump sensing brackets as described above. Access to 18% to 13.5%, respectively, of the examination volume is restricted and access to the outside surface is not possible due the Sacrificial Shield (Bio-shield) surrounding the Reactor Vessel. The remaining 82.01 % and 86.65%, respectively, of the examination volume was examined with techniques which have been qualified by demonstration in accordance with ASME Section XI, Appendix VIII, Supplements 4 and 6 of the ASME Section XI, 2001 Edition through the 2003 Addenda, using the Performance Demonstration Initiative Protocol. This weld was examined from both sides of the weld, in both parallel and perpendicular directions. The ultrasonic examinations revealed no indications in either weld.

Examination Category 8-A, Item 81.22 Figures 11 through 12 illustrate the weld locations and the location of the areas of incomplete coverage. The examination volume requirement depicted per Figure IW8-2500-3 is limited due to the obstruction from the RPV skirt support and CRD penetrations.
RPV Head-to-Flange Weld (HCC-C001)
Examination Category B-A, Item 81 .40 The RPV head-to-flange weld HCC-C001 is examined from the head side only due to head-to-flange configuration. Figures 14 and 15 depict the scan limitations and the location of the areas of incomplete coverage.
This examination consisted of the weld circumference between stud holes 40 to 60 only. The examination volume requirement depicted per Figure IW8-2500-5 is limited to 76.05% due RPV head-to-flange configuration as this weld is examined in 113rd sections each ISi period.
The Nondestructive Examination (NOE) procedure used for this examination incorporates the examination techniques qualified under Appendix VIII of the ASME Section XI Code by the Performance Demonstration Initiative (POI). That procedure was approved under Relief Request NOE-ROOS on January 31, 2007.
RPV Nozzle-to-Shell Weld (FWD-DOOt LCA-0001, MSC-0001 and Vl8-D001)
Examination Category 8-0, Item 83.90 Figures 16 through 19 illustrate the scan limitations and the location of the areas of incomplete coverage of welds FWD-0001, LCA-0001, MSC-0001 and Vl8-D001. The examination volume requirement depicted per Figure IW8-2500-7(b) is limited to single side access due to the RPV nozzle-to-shell configuration.
RPV Threads in Flange (Ligaments (Stud Holes 1-60))
Examination Category 8-G-1, Item 86.40 The manual scanning of the 1 inch annulus surface around the RPV flange stud hole 1 through 60 is performed from the RPV flange surface. Figure 20 provides an illustration of the RPV flange surface limitation due to interference from a machined step on the flange surface where the 0-rings are seated. This causes a loss of contact for the 0-degree UT examination.
The examination volume depicted per Figure IW8-2500-12 is limited when scanning the carbon steel base material surrounding the stud holes due to a machined step which prohibits access to approximately 12.85% of the required examination volume. The accessible 87.15% of the examination volume was examined with ASME Code acceptable techniques. The ultrasonic examination did not reveal any recordable or reportable flaws in accordance with the ASME Code Section XI, 2001 Edition through the 2003 Addenda.

Examination Category C-B, Item C2.21 The examination of Figure 21 shows the scan limitations and the location of the areas of incomplete coverage. The examination volume depicted per Figure IWC-2500-4(b) is limited due to interference from the adjacent shell to flange weld.
- 5. Burden Caused by Compliance It is not possible to obtain ultrasonic interrogation of greater than 90% of the required examination volume due to interferences caused by configuration, access restrictions and/or permanent attachments. All examinations were performed to the maximum extent possible. The ultrasonic (UT) techniques for each weld or surface were reviewed to determine if additional coverage could be achieved. For the welds or surfaces listed above, it was not possible to remove the obstruction without significant work, increased radiation exposure, and/or damage to the plant.
- 6. Proposed Alternative and Basis for Use Proposed Alternative
- 1) Periodic system pressure tests in accordance with ASME Section XI Category B-P, Table IWB-2500-1 and ASME Section XI Category C-H, Table IWC-2500-1.
- 2) Conduct ultrasonic examinations to the maximum extent possible.
NextEra Energy performed inservice examinations of selected welds and surfaces in accordance with the requirements of 10CFR50, Section 50.55a, plant Technical Specifications, and the American Society of Mechanical Engineers (ASME) Boiler and Pressure Vessel Code,Section XI, 2001 Edition through the 2003 Addenda. When a component has conditions that limit the examination volume, NextEra Energy is required to submit the information to the enforcement and regulatory authorities having jurisdiction at the plant site. This Relief Request has been written to address areas where those types of conditions exist and the required amount of coverage was reduced below the minimum acceptable.
10 CFR50, Section50.55a(g)(4) recognizes that throughout the service life of a nuclear power facility, components which are classified as ASME Code Class 1, Class 2, and Class 3 must meet the requirements set forth in the ASME Code to the extent practical within the limitations of design, geometry and materials of construction of the components.
NextEra Energy performed the examinations to the extent possible. There is no plant-specific, NextEra fleet or industry operating experience regarding potential degradation specific to the subject welds included in this relief request.
In the unlikely event that a through wall flaw were to occur, Leakage monitoring inside containment at Duane Arnold unit 1 is provided by the reactor cavity (containment) sump

This system has high level and alert status alarms in the control room. This system has Tech Spec required monitoring (TS 3.4.4.1) at least once every 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.
NextEra Energy performed ultrasonic examinations of the reactor vessel welds required to be examined in accordance with the requirements of 10 CFR50 Section 50.55a, plant Technical Specifications, and the ASME Section XI, 2001 Edition through the 2003 Addenda to the maximum extent possible. Additionally, the mechanized techniques employed for examination from the RPV inside surface have been demonstrated in accordance with Supplements 4 & 6 of the ASME Code Section XI, Appendix VIII, 1995 Edition through the 1996 Addenda, using the Performance Demonstration Initiative Protocol. The Ultrasonic (UT) techniques for each weld were reviewed to determine if additional coverage could be achieved for those areas limited from the inside surface. Access limitations due to permanently installed attachments inside the Reactor Vessel limited scanning of the welds included in this request for relief. Access for examination of the affected vertical welds from the outside of the reactor vessel is not possible due to the Sacrificial Shield (Bio-Shield). In addition to the required ultrasonic examinations, the interior of the Reactor Vessel, including welded attachments, received a visual examination in accordance with Table IWB-2500-1, Examination Categories B-N-1 and B-N-2. The VT-1 and VT-3 visual examinations revealed no relevant indications.
The subject welds were examined during the Fourth 10-year ISi Interval Reactor Vessel examination. No flaws were identified during the examination.
The extent of examination volume achieved ultrasonically and the system pressure tests provide assurance of an acceptable level of quality and safety.
- 7. Duration of Proposed Alternative This relief request is applicable to the Duane Arnold Unit 1 Fourth lnservice Inspection Interval which began November 1, 2006 and ended October 31, 2016.

The limitations at the top of the inspection area were due to the horizontal scan rail contacting the underside of the jet pump restraint brackets. See Figure 8 (VS-11-01 & VS-11-02)
- The limitations in the middle area of the weld were due to a bracket mounted to the jet pump. These brackets are installed in the field to secure the sensing lines along the side of the jet pump; the orientation limited the scanner travelling vertically. See Figure 9 (VS-11-06)
- The limitations at the bottom of the welds were due to the sensing lines and brackets installed at the bottom of the jet pumps as well as the Jet Pump support plate. The brackets and sensing lines interfered with the cables on the rear of the scanner preventing full travel. See Figure 10 (VS-11-07<1 > & VS-11-07<2>)

The Ijl course longitudinal welds are limited in examination coverage due to the proximity of the jet pump support plate: jet pump restrainer bracket and sensing line bracket. Scanning was conducted adjacent to the obstructing lugs with the scan boundaries maximized by visually assisted positioning of the exam sled. Coverage was achieved to the maximum extent practical by using solid modeling to design the robot scan routines around the limitations. Extra transducers were also added to the exam sled to maximize coverage. Final examination cowrngc achieved is 82.01% and 86.6% respect1vd~
