The following information was received via E-mail:
The licensee's Radiation Safety Officer reported on June 7, 2019 that the licensee discovered on June 6, 2019 that 31 of 34 Fe-55 sealed sources tested positive for Fe-55 contamination upon performance of leak testing of the sources in preparation for disposal of the sources. Six (6) of the 34 sealed sources exceeded reporting limits of 0.005 microCuries of removable contamination for each source. The highest removable contamination for a source was reported to be 0.025 microCuries.
All sources were used sources returned by customers accumulated by the licensee over a period of several years. The licensee is not certain at this time whether only one or more of the sources is actually leaking and contributed to the contamination of the other sources, all in the same storage container.
The sources have various manufacturer and model numbers. Most of the sources have a current activity of below one (1) millicurie and the highest activity source is currently approximately six (6) milliCuries.
All of the sources are secured in a lead storage container, inside a safe, in the licensee's source room, pending appropriate packaging and disposal of the sources. Area surveys were performed by the licensee and the licensee reports that there is no contamination outside of the storage container and no personnel exposures resultant of this event.
The licensee will provide a written report to the Agency [MA Radiation Control Program] in accordance with condition of its license within 5 days. The Agency considers this event to be open.
