ENS 459959 June 2010 17:38:00The following information was obtained from the State of Utah via email: Two tritium exit signs were purchased and installed between March 2004 and October of 2005 by Shaw Environmental, Inc., as part of a contract with the United States Army Corp of Engineers (performing) renovation of military recruiting centers. Shaw is conducting an inventory of all tritium exit signs purchased and installed under this contract and identified that the listed signs are not located where installation records indicated that they were installed. Manufacturer: SRB Technologies, Inc.; Model: BetaLux-E, each contains 10 Ci of H-3, Serial Numbers 000102 ((previously located at) 175 S. Main St., Salt Lake City, UT) and C000108 ((previously located at) 545 E. 4500 St., Ste. E-125, Salt Lake City, UT) This is the third report of missing tritium exit signs discovered during Shaw Environmental, Inc.'s inventory effort. See EN #45972 and EN #45885. THIS MATERIAL EVENT CONTAINS A "LESS THAN CAT 3" LEVEL OF RADIOACTIVE MATERIAL Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks.
