The missing caps effectively i ITTT1 I caused the outer hatch door to be bypassed by a 3/4" connection.

The t oisi i immediate actions were to reinstall the caps.

The emergency doors were ;

Ioperable.

There have been no similar occurrences.

LER 50-313/80-040 ANO-Unit 1 Cause Description & Corrective Actions:

The caps were replaced and all other hatches, Unit I and II, were verified secured.

The following actions were assigned to prevent reoCCurrenCO.

1.

The local leak rate test procedures are to be revised to require specific signoff steps assuring that the test connections are isolated.

2.

Plant preheatup containment integrity checklists are to be revised to include local leak rate connections on hatches.

3.

Design changes will be initiated as necessary to install isolation valves for local leak rate test connections.
