SUBJECT: DAVIS-BESSE INSPECTION REPORT 50-346/98017(DRP) AND
Dear Mr. Wood:

On November 9,1998, the NRC completed an inspection at the Davis-Besse reactor facility.
During that inspection, plant operators were challenged by several events which led to manual -
reactor trips from approximately full power on September 24 and October 14, an automatic
,
reactor trip from 4 percent power on October 18, and a plant runbcck from 100 percent to
i- 60 percent power on October 21. The circumstances surrounding the October 14 manual
L reactor trip were described in Inspection Report 50-346/98019._ The circumstances surrounding 'i

I ; Operators responded promptly and effectively to the events to ensure the plant was in a stable
' condition following each event. While each of these events were reviewed and individual causes
were identified and addressed, it is of concern that the processes in place were not effective in
- preventing these events from occurring. It is our understanding that a collective significance
i review has been initiated to assess the various events that have occurred over the past several
'
months and to develop actions to improve overall performance at the facility. A management
meeting to discuss the initial results of the review has been scheduled for December 16,1998,
at the NRC Region lli Office in Lisle, Illinoisc ,
One' violation of NRC requirements was identified during this inspection conceming an
. inadequate maintenance work order (MWO) which was used by an electrician during the
, removal of a primary water storage tank temperature indicator. The MWO contained incorrect
'~
instructions concerning a fuse removal from an instrumentation panel and, when the work was
performed, the plant runback mentioned above occurred.
The violation is cited in the enclosed Notice of Violation (Notice) and the circumstances
= surrounding it are described in detail in the enclosed report. The NRC has concluded that
information regarding the reason for the violation, and the corrective actions taken and planned
.
to correct the violation and prevent recurrence, are already adequately addressed in the
- enclosed report. Therefore, you are not required to respond to this letter unless the description

therein does'not accurately reflect your correct lve actions or your position, in that case, or if you
- choose to provide additional information, you should follow the instructions specified in the
