Subject: Inspection Report No. 50-271/86-08
This refers to the routine resident safety inspection conducted by Mr. W. J. Raymond
of this office on March 18 - May 5, 1986, at the Vermont Yankee Nuclear Power Sta-
tion, Vernon, Vermont. The inspection findings were based on observation of acti-
vities, interviews with personnel and document reviews. The results of the in-
spection are described in the NRC Region I Inspection Report enclosed with this
letter and were discussed with Mr. J. P. Pelletier of your staff at the conclusion
of the inspection.
Based on the results of this inspection, it appears that one of your activities
was not conducted in full compliance with NRC requirements, as set f)rth in the
Notice of Violation, enclosed herewith as Appendix A. The violation concerns the
failure to complete adequate corrective actions to identify and control defective
NAMCO contact blocks to prevent inadvertent use or installation, following the
discovery of defects in the blocks that were reported by your letter dated 3/28/86
under 10 CFR 21 as a matter that constituted a potential safety hazard. This
violation has been categorized by severity level in accordance with the revised
NRC Enforcement Policy (10 CFR 2, Appendix C), published in the Federal Register
on March 8, 1984 (FR 8583). You are required to respond to this letter and in
preparing your response, you should follow the instructions in Appendix A.
An additional item discussed in Section 7 of the enclosed inspection report con-
cerns the adequacy of escort control provided by your recirculation piping con-
tractor on February 13, 1986 for an ironmaker, when the individual was brought on-
site under a visitor status for a weldiig test. At the request of our resident
inspector, site security reviewed this matter and determined that the individual
was most likely either under direct escort control or under surveillance by badged
personnel while on site. We have no further questions regarding this matter at
the present time. However, we encourage you to ensure that plant and contractor
personnel responsible for escorting visitors follow security procedures diligently.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice", a copy of this
letter and the enclosures will be placed in the NRC Public Document Room.
