Good Morning Mr. Gonzalez,
Thank you for taking the time to review my request to renew this license. I've just been notified that we
are going to close the location this week. Once I have the sources of radiation removed and a close out
survey completed, I'll be submitting to term the license.

This is to provide notification of the intent to decommission the Diagnostic Health Anchorage, LLC (a
wholly owned subsidiary of Alliance HealthCare Services) Radioactive Materials License 50-23214-01.
We are in the process of arranging for disposal of the sealed sources at the location. Once that is
completed, I will submit to you the closing documents.
