ATTN: Jack Dauch President 219 Fremont Avenue Sandusky, OH 44870 Gentlemen:
We have reviewed you' r application dated September 27, 1985 requesting a new license to perform leak testing service. We find that we need additional information concerning your program. The NRC has prepared the " Guide For the Preparation of Applications For License For the use of Radioactive Materials in Leak-Testing Services" to assist applicants in providing necessary information. Refer to the enclosed guide and provide the following additional information:
- 1. Provide a more detailed description of the element and mass number of the radionuclides involved in performing the leak test service. Refer to the guide, Item 5 on page 5 for examples of suggested responses.
- 2. Describe the kinds of sealed sources or sources in devices to be leak tested. For example, specify the radioisotope and the amount in curies.
Refer to Item 6 on page 6 of the guide for specific examples of the description needed.
- 3. Regarding the training provided to any persons involved in leak checks, please provide the information requested in the guide in Item 8 on page 9.
Briefly, this includes providing an outline of the radiological health topics covered, name and training of instructors and describing records of ,
training.
- 4. Provide a more complete description of your laboratory facilities located at 219 Fremont Avenue, Sandusky, Ohio. Include a description of the items addressed in Item 9 on page 10 of the guide, i 5. It appears form your application that personnel will . wear TLD whole body and finger badges as well as pocket dosimeters during collection of the leak test sample. Therefore, please specify that the organization i furnishing the TLD service will be a commercial service company and state l the exchange frequency for the TLD's. i
- 6. In addition to the above, describe the use and calibration of the pocket i dosimeters. We recommend that the dosimeters be recharged to zero prior to each days use and be calibrated at intervals not to exceed one year.

- 7. Describe the calibratten of your survey meters including frequency and method. Refer to Item 10.2.1 on page 11 of the guide.
- 8. Describe the calibration of ycur wipe sample counting equipment. Refer to Item 10.2.2 on page 13 of the guide.
- 9. Modify your radiological safety program to include the following:
- a. Any specific instructions provided by the source and device manufacturers on recommended methods and areas to be wiped. ;
- b. Instructions on what to do in case of emergencies. Include tle items addressed the the guide in Item 10.3 on page 14. l Submit the modified program.
- 10. Describe the types of records you will maintain on leak test samples. f These records should include the items addressed in Item 10.5 on page 15 i of the guide. '
