At 1400 hours on February 19, 1977, while performing a surveillance test on the HPI flow transmitters, it was discovered that the two flow transmitters on Loop "A" (PDT-1210 and PDT-1209) had failed and the two flow transmit.ters on Loop "B" (PDT-1228 and PDT-1230) were out of the recommended manufacturers tolerances.                                                           -

The bellows of. PDT-1210 and PDT-1209 had failed, therefore the flow indicator would not have indicated flow upon the actuation of an ES signal. PDT-1228 would have indicated flow 1% low and PDT-1230 m               would have indicated 4% low upon actuation of an ES signal.
