DESCRIPTION OF DEFICIENCY The deficiencies were discovered when construction electricians were terminating field cables in Motor Control Centers (MCC).

Further investigations were performed to determine if the deficient terminations were isolated to just one MCC or were present in many of our safety and non-safety related MCCs. A partial survey of MCCs indicated that the problems appeared to be fairly widespread.
