Dear Mr. Grier:

l l On December 4, 1980, in accordance with 10CFR50.55 (e), we l reported verbally to Region 1 a deficiency with cable terminations in Square D supplied 480VMotor Control Centers. This letter

. serves as our 30 day written report of this deficiency.

DESCRIPTION OF DEFICIENCY The deficiencies were discovered when construction electricians were terminating field cables in Motor Control Centers (MCC).

Further investigations were performed to determine if the deficient terminations were isolated to just one MCC or were present in many of our safety and non-safety related MCCs. A partial survey of MCCs indicated that the problems appeared to be fairly widespread.

The problems found appear to be caused by a lack of adequate quality control in the method in which cable terminations were made in both safety and non-safety related MCCs. Termination lugs on vendor wiring have been found insufficiently crimped such that conductors can fall out of or easily be pulled out of their lugs. Other cases have been found where terminal block screws and screws on devices have not been tightened to secure the termination lugs in place. Mr. Boyce H. Grier December 30, 1980 Page two have been bent further than permitted by the lug manufacturer and where the conductor has not been properly inserted into the lug before crimping. These defic 2incies can cause intermittent or broken electrical circuits whion might cause safety related equipment to fail to operate when required.

CORRECTIVE ACTION Square D engineering and quality assurance have been appraised of this problem and a Square D quality assurance representative has visited the site to inspect samples of the deficient termin-ations.

Field electricians have removed and replaced deficient terminations in several of the MCCs that have been inspected. We are presently awaiting a response from Square D as to the results of their on site inspection before a full plan of corrective action is under-taken.

time, a supplemental report will be submitted in 90 days. Final l Corrective Action, Corrective Action to Prevent Recurrence and Date l of Full Compliance will be provided at that time.
