Confirming our telephone conversation of August 16, 1989, in which we discussed four questions raised in your letter of July 31, 1989, the following is a summary of the answers to the question: Is it necessary to obtain an amendment to your license under the following conditions:

Confirming our telephone conversation of August 16, 1989, in which we discussed four questions raised in your letter of July 31, 1989, the following is a summary of the answers to the question:

Is it necessary to obtain an amendment to your license under the following conditions:

Change in the nome of the trained user (due to marriage or divorce, etc.)

Answer:

Yes, technically, a change in a person's name should be reflected in the license.
