Gentlemen:
Confirming our telephone conversation of August 16, 1989, in which we discussed four questions raised in your letter of July 31, 1989, the following is a summary of the answers to the question:
Is it necessary to obtain an amendment to your license under the following conditions:
1.
Change in the nome of the trained user (due to marriage or divorce, etc.)
Answer:
Yes, technically, a change in a person's name should be reflected in the license.
2.
The termination of employment of any of the individuals named in Section 12A.
Answer: Yes, if the person has been designated as the Radiation ProtectionOfficer(RPO). No, if persons other than the RP0.
3.
A change in job duties or location such that it is more logical for a trained person other than Roger Marble to perform the duties of RP0.
Answer:
Yes.
4.
The training of addition people by Lixi Inc. in the use of our Lixi Scope.
Answer:
Yes, under the current license an amendment would be required.
However, an amendment could be submitted for approval which would permit persons (not named in the license) to use the Lixi Scope under the direction of the RP0 after they have received appropriate training so long as records of training is kept available for inspection.
