Enclosed is a copy of the Final Report for the Seabrook Station Plume and Post Plume Biennial Exercise, conducted on April 17-18, 2012.

The Commonwealth of Massachusetts, State of New Hampshire, State of Maine and local emergency response organizations successfully demonstrated their capabilities to implement their off-site radiological emergency response plans and procedures based on the evaluation of this exercise by a team of Federal evaluators with final determinations made by the Regional Assistance Committee (RAC) Chair.

There were four Areas Requiring Corrective Action (ARCA) as a result of this exercise. There were no deficiencies. There are four open ARCAs as a result of this exercise: one due to Emergency Operations Facility not providing adequate instructions to Field Teams. The three remaining ARCA's at Kingston, Newfields and Portsmouth Emergency Operations Centers were that the Radiological Officers failed to provide adequate dosimetery briefings to Emergency Workers. State and local preparedness remains adequate to protect the health and safety of the public living in the vicinity of the Seabrook Station and provides reasonable assurance that appropriate measures can be taken off-site in the event of a radiological emergency.

SUMMARY

On April 17, 2012 and April 18, 2012, the U.S. Department of Homeland Security (DHS),

Preparedness Directorate, National Preparedness Division, Radiological Emergency Preparedness (REP), Federal Emergency Management Agency (FEMA) Region I conducted an exercise in the 10-mile plume exposure pathway emergency planning zone and the 50-mile ingestion exposure pathway emergency planning zone around the Seabrook Nuclear Power Station. Interviews and out-of-sequence demonstrations for schools, other special facilities, and reception, monitoring, and decontamination centers (as outlined in this report) were conducted within 60 days of the exercise. The purpose of the exercise and out-of-sequence activities was to assess the level of State and local preparedness in responding to a radiological emergency. This exercise was held in accordance with FEMA's policies and guidance concerning the exercise of State and local radiological emergency response plans (RERP) and procedures.

DHS wishes to acknowledge the efforts of the many individuals in The State of New Hampshire, The Commonwealth of Massachusetts, State of Maine, local communities, and private and volunteer organizations that participated in this exercise.

Protecting the public health and safety is the full-time job of some of the exercise participants and an additional assigned responsibility for others. Still others have willingly sought this responsibility by volunteering to provide vital emergency services to their communities.

Cooperation and teamwork of all the participants were evident during this exercise.

This report contains the final evaluation of the biennial exercise and the out-of-sequence activities.

SUMMARY

2.1 Exercise Purpose and Design The FEMA Region I evaluated the exercise on April 17, 2012 and April 18, 2012, to assess the capabilities of local emergency preparedness organizations in implementing their Radiological Emergency Response Plans (RERPs) and procedures to protect the public health and safety during a radiological emergency involving Seabrook Nuclear Power Station. The purpose of this report is to present the results and findings on the performance of the offsite response organizations (OROs) during a simulated radiological emergency in the HSEEP format.

2.2 Exercise Objectives, Capabilities and Activities 2.3 Scenario Summary The exercise scenario was developed to evaluate the response of the exercise participants to a radiological emergency. The scenario is listed in Appendix: Exercise Plan.

After Action Report/Improvement Plan Seabrook Station SECTION 3: ANALYSIS OF CAPABILITIES 3.1 Exercise Evaluation and Results This section contains the results and findings of the evaluation of all jurisdictions and functional entities that participated in the April 17, 2012 and April 18, 2012 Ingestion Plume Exercise, conducted to test the offsite emergency response capabilities of State and local governments in the Seabrook Nuclear Power Station 10-Mile and 50-Mile Emergency Planning Zone.

Each jurisdiction and functional entity was evaluated on its demonstration of criteria contained in the exercise evaluation areas as outlined in the federal Register, Volume 67, No. 80 "FEMA -

Radiological Emergency Preparedness: Exercise Evaluation Methodology" (April 25, 2002).

Detailed information on the evaluation area criteria and the extent-of-play agreements for the drill are included as appendices to this report.

3.2 Summary Results of Exercise Evaluation The matrix presented in the table on following pages presents the status of all exercise evaluation area criteria that were scheduled for demonstration during the drill by all participating jurisdictions and functional entities. Exercise criteria are listed by number, and the demonstration status of those criteria are indicated by the use of the following letters:

Emergency workers periodically and at the end of each mission read their dosimeters and record the readings on the appropriate exposure record or chart. The RADEF Officer did not understand his role in providing a briefing on dosimetry and potassium iodide (KI)to emergency workers before dispatching them to the field.

He issued the dosimetry and KI without a briefing.

At 1130, during the Site Area Emergency (SAE) Emergency Classification Level (ECL) the RADEF Officer performed an Emergency Worker (EW) dosimetry briefing for three ambulance workers at the Kingston EOC. After this briefing, two of the EW were interviewed to determine their basic knowledge of radiation exposure limits, as specified, their knowledge of procedures to monitor and record dosimetry readings, and their ability to manage their radiological exposure. During this interview the EW were not able to correctly identify their first reporting level of 175 73

POSSIBLE CAUSE: The briefing provided by the RADEF Officer consisted of his reading from the Emergency Worker Information sheet, New Hampshire Dosimetry Equipment and Procedures Vol.8/Rev. 14, 10.8-1 to 10.8-6. It was determined by interview that the RADEF Officer was not specifically trained in this EOC staff position, but was assigned it on his arrival at the EOC. He therefore relied solely on the Emergency Worker Information sheet which was laid out in a way that confused both the RADEF Officer and the EWs as to administrative and turn-back limits.

POSSIBLE CAUSE: At 1201, Kingston's Emergency Management Director (EMD) confirmed that the State of New Hampshire recommended that all EW take KI within the Emergency Planning Zone. This was relayed to the RADEF Officer who occupies an adjacent room within the facility. The RADEF Officer and his assistants distributed four KI tablets to EOC staff per their plans and procedures. At 1241, the RADEF Officer conducted a KI briefing in the EOC proper and staff was directed to simulate taking one KI tablet.

The EOC is located on the second floor of the Kingston Fire Station. Along with the EOC room, a radio room (with three staff) and EOC Security at the stairs (with two staff) are also located in the facility. After issuance of KI to staff at 1241, again at 1319, and at the end of the exercise at 1339 it was determined by interview that the radio room and security staff had not received KI tablets or been briefed on KI use.

Furthermore, the Radiological Officer provided inaccurate information pertaining to women and did not request that female workers who may be occupationally exposed (i.e., those who work in the restricted area at a nuclear power plant or at a hospital or other facility and whose occupation carries with it the potential for some radiation exposure) are counseled to make a declaration in writing if they are or think that they may be pregnant.

The Radiological Officer also had a Job Aid that conflicted with the Briefing Form that the State provided to the Emergency Operations Center.

The Radiological Officer was also using Rev 13 for Form 305A, which is out of date.

These two lines served the 22 people signed into the EOC who were making out going calls. It was discovered no calls could come into the EOC. As a fix one line was designated as an incoming line and one as an outgoing line. However, there were still individuals who tried to call in and, unknown to the EOC, were never able to get through to the EOC.

CONDITION: There was conflicting guidance given to vehicle Monitoring Team at the Manchester, NH Reception Center on the morning of March 24, 2012. The team members were confused as to which guidance was to be used. Controller and team members consulted the evaluator as to what guidance to use. As the team was initially were conducting monitoring of a vehicle, two team members were employed at the same time and both were using different heights from monitored surface and different speeds in inches per second over the vehicle surface. As just in time training and correction in place the evaluator instructed the team to proceed with the most conservative of the three different standards.

POSSIBLE CAUSE: This evaluator was provided "April 2011, Radiological Emergency Response for Nuclear Facilities Attachment C, Implementing Procedures for Host Communities" on Table #2 the monitoring instructions for the Victoreen CDV-770-P "pancake probe" indicate "4 inches from the surface being monitored and a speed of 24 inches per second".

The Vehicle Monitoring staff had tow documents "5.7 Staff- Vehicle Monitoring Rev. 1/1/2012" which stated hold probe"l inch from surface and the vehicle monitoring should take 90 seconds" with no mention of monitoring sweep speed in inches per second.

The second document "Vehicle Monitoring 10-10 states to "hold the probe at 2 inches. Do not move fast!" and "monitoring of vehicle should not take less than 90 seconds". Again no mention of probe speed in inches per second.

FMT 1 never received direction to find the leading edge of the plume. The sample locations of 3.2, 3.7 and 3.8 provided by the Monitoring Team Coordinator (MTC) to the FMT I Team Lead never allowed FMTI to encounter any reading above background. Both air samples taken by FMT I were taken in background areas.

RECOMMENDATION: The Accident Assessment team and MTC should coordinate more effectively to direct both FMTs to identify the leading edges of the plume. The plans and procedures should reflect the need to identify the leading edges of the plume. Identify in the plans and procedures what actually constitutes the leading edges of a plume. For example when FMTs identify dose rates 10 times background this will be considered the leading edge of the plume.

The Radiological Officer summarized the briefing using the Job Aid and gave inadequate information to emergency workers. The Radiological Officer's Job Aid conflicted with the Briefing Form the State provided to the EOC, which contains all information needed for emergency worker exposure control. The Radiological Officer also skipped important information that emergency workers needed to be briefed on prior to being deployed to the field (the Radiological Officer did not read off the Briefing Form that the State had provided to all EOCs)- the Radiological Officer did not address the 175mR exposure limit; only stated to notify the EOC when the dosimeter reaches IR. Furthermore, the Radiological Officer provided inaccurate information pertaining to women and did not request that female workers who may be occupationally exposed (i.e., those who work in the restricted area at a nuclear power plant or at a hospital or other facility and whose occupation carries with it the potential for some radiation exposure) are counseled to make a declaration in writing if they-are or think that they may be pregnant.

The Radiological Officer also had a Job Aid that conflicted with the Briefing Form that the State provided to the Emergency Operations Center.

The Radiological Officer was also using Rev 13 for Form 305A, which is out of date. Rev 14 Forms are the most up to date and and FEMA approved forms for emergency workers.

The Emergency Operations Center also had the wrong inventory form for the emergency worker kits. The one provided was out of date.

RECOMMENDATION: This previous issue was not corrected.

Training needs to be provided to the Radiological Officer. Rev 13 Forms need to be destroyed. The Job Aid must align with the Incident Annex and Implementing Procedures, as well as the City's Radiological Emergency Response Plan.

Supply Issue: Procedures call for maslin mops to mop walk ways to pick up contamination that may have dropped on the floor. The male monitoring and decontamination team had mop heads and handles but did not have any maslin cloth. The Supply trailer was checked but no maslin cloth could be found.

RECOMMENDATION: Take steps to procure the correct type step off pads and maslin cloth with sufficient quantities to support long term operations. Update inventory lists referenced above to include specific type step off pads and add mop-heads and handles along with maslin cloth.

He noted that the town was now including children between the ages of 10-15 in the special population group, that is, children who were decision makers but who needed assistance in evacuating the town. In a specific case, there was a 14 year old who was home alone - her mother was working in Portsmouth. In her case, she knows that she needs to evacuate but she cannot drive a car.

Therefore, the EMD would be sure to include her in the population to be transported. This is definitely an underserved population.

2. York County Emergency Management Agency EOC and facility modernization Summary: York County relocated its Emergency Management Agency and Emergency Operations Center (EOC) location to a new site at 149 Jordan Spring Rd., in Alfred, ME within the past year and, as a result, created a facility that is an excellent model for any EOC supporting an agency of comparable size. The facility has excellent space available, security, equipage and habitability for extended EOC operations as well as routine activities. It is evident that significant appropriate planning was done to accommodate virtually any weather event durability-wise or other types of emergencies requiring staffing of almost any level that can be anticipated for the Agency. This includes providing effective backup power, upgrading windows and air handling systems and making the best use of spaces available.

One TV screen displayed door status throughout the facility and a second TV screen display security cameras, some of which were movement activated.

Maine (MEMA) communications office coordinated with MEMA (Massachusetts) communications officer having similar problems with talking to Maine Emergency Management.

Mass provided a radio to Maine which now allowed Maine to connect with the Amesbury, Mass repeater. This now allowed Mass to communicate with Maine and allowed Maine to communicate with five other New England states.

Maine RACES: installed a repeater on Mount Washington this now provided an ability to communicated with the New Hampshire SEOC.
