Thefanswerenotoperablethroughmanualorautomatic controlandthethermaloverloads werefoundtripped.Thethermaloverloads werereplacedwithqualified inkindpartsandtestedbymanuallystartingthefanthreetimes.Aone-hourtestrunofthefan(3MA275)wasperformed onthethirdstart.5.Thiseventoccurredduringaperiodofinoperability previously scheduled formaintenance activities.

Arevisiontotheprocedure willindicatethattheradiatorfanbreakersshouldbetaggedopenedwhentheDCcontrolpoweristaggedopened.Theresponsible individuals werecounseled ontherequirements formanipulating safetyrelatedequipment.

AnOperating DivisionExperience Report(2-90-22) hasbeenpreparedinresponsetothisevent.5.Thisfailureoccurredduringaperiodofinoperability previously scheduled formaintenance activities.
