1. The Utah site access permit is a rad waste permit, I need to contact RP at the site and then I will get that info for you.

2. New DOT Haz Mat certification is attached.

3. The WPDES permit is still in review. The WDNR has indicated that our last permit is still in effect until the submitted application is acted upon.

For the following permits, could you email me copies of the renewed licenses and expiration dates for the following:

Registration (49 USC 5108, Hazardous materials shipments)

WPDES permit Utah site access permit If you do not have a renewed permit but submitted a timely renewal, please let me know. None of this has to be done under oath and affirmation.
