  • Narrative Report for LER 50-321/1981-037

- On 4-21-81, with Unit 1 in cold shutdown for a refueling and torus' modification outage, a laboratory analysis of a charcoal-sampic taken frou control room filter train Z41-D004B was found to remove 77.9% of methyl iodide instead of the 90.0%

requited by llNP-1 Tech Specs 3.1.2.A.2 and llNP-2 Tech Specs 4.7.2.C.2.. The faulty charcoal was removed and rep? aced . by new charcoal. The new charcoal was tested por HNP-1-3975,

" Testing of Control Room and SBGT Filter Train by Vendor", and passed the DOP and Icakage test. A charcoal sampic was sent offsite for a laboratory analysis for methyl iodide removal.

The charcoal passed the test with 99.4% methyl iodido remov-al. The failure of the original charcoal was apparently due to paint fumes passing through . the filter trains while the control room ventilation system was in the recirculation mode. 1Z41-D004A was tested on 3-19-81, and was certifica according to llNP-1-3975 ~on .4-22-81. This is a nonrepetitive event for this system with no effects upon public health and safety.
