MODIFICATION 1 TO APPLICATION AMENDMENT N-1 With reference to activities authorized by SNM-1097, on 8/3/78 General Electric Company requested authorization to use a particular canister with full face masks which is currently being evaluated for approval by the National Institute of Occupational Safety & Health (NIOSH). The canister identification given in this request was incorrect.

The correct identification for the canister is 88182 GMR, to be used with the MSA chin-type gas mask with Ultravue or Clearvue Face Piece #457111.

With respect to the request by your staff for information related to the application made by the Mine Safety Appliance Corporation (MSA) to NIOSH, attached is a copy of that application letter including references to the actual d,ata supplied by NIOSH.

Your staff further requested that a copy of the data sent to NIOSH in support of the attached application also be submitted in order to complete evaluation of our request. Therefore, we will request a copy of the data from MSA and forward to you upon receipt.

These GKR-C Gas Fbsks assemblies have been subjected to the required tests i

described in Subparts I and K of Title 3D CFR, Part 11. The following tables of dato are attached; Table 4A - 0.5% Chlorine Tests s

Table 4D - Silica Dust Tests Table 4C - DOP Tests The following tables of data, which are relevant to all of the assemblies are attached:

Tablo 5 - DOP Han Work Testo, Clearvue Facepiece Table 6 - DOP tbn Work Tosts, Ultravue Facepiece Table 7 - Exhalation Valve Tests, Ultravue and Cicarvue Facepieces An engineering package of drawing lists, drawings and specifications not previously cubmitted on other packages, and test data is attached for each of the assemblics listed in Sections 1 through 4.

The basic clearvue f ace-piece, 7-223 and the banic ultravue facepiece, 7-203 were previously approved by NIOSH. The canisters are new approvals.

The quality control procedures (C/D'a) have not been completely written and will be nupplied by Quality Control when they are available.

Those which are available and have not been previously submitted are attached to the package.

All parto of the gas mask assemblies are production parts.

Samples of canisters being submitted to'NIOSil for approval are available from Engineering.

Please advise the feo required for this approval and if any additions 1 information is needed, please contact the undersigned.
