In Section'II, note "No. 5" on page 5 should be modified (II) to read as follows:

"Programatic changes (except those that are editorial in nature) in the QA program as described herein shall require written NRC notification prior to implementation.

Orgnizational changes which affect the QA program will be reported to the NRC within 30 days after announcement."

In Appendix B (Exceptions), item "C" on page 8 should bc (App. B) modified to read as follows:

"The Yankee Nuclear Services Division Radiation Protection Group shall at least include an individual that possesses a bachelor's degree or the equivalent in a science or engineering subject, including formal training in radiation protection and at least five years of professional experience in applied radiation protection. At-least three years of this professional experience should be in applied radiation protection work at an operating nuclear power plant."
