dated May 15, 1986 In accordance with our referenced letter, this submittal provides the follow-up report of fire-rated assemblies (specifically fire doors) found inoperable during routine surveillances in May 1986.

i Fire doors found inoperable during the May routine surveillances are identified in the Attachment and include: 12 latching mechanism failures and 18 closure mechanism failures.

No doors were found blocked open in May. As each of these deficiencies was identified, compensatory fire watches, pursuant to Technical Specification 3.7.9, Action Statement

This monthly follow-up report and those sent previously, provide updated information associated with an ongoing condition initially reported in 1983.

The follow-up report covering inoperable fire doors identified in June 1986 will be submitted by July 15, 1986.
