It appears that you are a separate former onepeay from General Electric, and have taken over their operation.

Section 30.34(b) of 10 CFR Part 30 (copy enclosed) states in part, ". ... Me lieease issued or granted pursuant to the regulations in this part . . . mor any right under a license shall be transferred, assigned, or in any manner disposed of, either voluntarily or involmatarily, directly or indirectly, through transfer of control of any lisense to any person . . ." Please note the change.

We have a By-Product material license #13-15523-01. This license iI specifies that the By-Product material will be used by, or under the ,

supervision of William Moore, Helen Hudson, or Mary Mellis. '

Mary Mellis has left the employment of Wellman Thermal Systems Corp.

We would like to replace the name of Mary Mellis with Maxine Adams.

We would like to also add the name of Mike Wampler to the license.

He has been trained in the operation and safety aspects of our equip-  :

ment by Isovac Engineering, Glendale, California. A copy of the ,

certificate of completion of the course is attached.  ;
