dated August 15, 1986 In accordance with our referenced letter, this submittal provides the follow-up report of fire-rated assemblies (specifically fire doors) found inoperable during routine surveillances in August 1986.

Fire doors found inoperable during the August routine surveillances are identified in the Attachments and include: 10 latching mechanism failures, and 2 closure mechanism failures. No doors were found blocked open in August. As each of these deficiencies was identified, compensatory fire watches, pursuant to Technical Specification 3.7.9, Action Statement 'a', were established.

Door mechanism failures are being corrected on an "as found" basis.
