This refers to the inspection conducted by Mr. J. J. Petrosino of this office on April 14-18, 1986 of your facility at Buffalo, New York, the continuation of the inspection on April 28-30, 1986 at the Conax Florida Corporation in St.
Petersburg, Florida, and to the discussions of our findings with R. E. Farchmin and members of your staff at the conclusion of the inspection.
The purpose of this inspection was to review the circumstances surrounding the-failure of the Conax explosive primer chamber device to perform its intended I, safety-related function at the Vermont Yankee nuclear station during a periodic test. Areas examined during the inspection and our findings are discussed in the enclosed report. Within these areas, the inspection consisted of an exam-ination of procedures and representative records, interviews with personnel, and observations by the inspector.
During the inspection it was found that the implementation of your QA program failed to meet certain NRC requirements. Inadequate implementation of your QA procedures was apparent in the areas of indoctrination, control of special processes and safety-related activities, and in the lack of a 10 CFR Part 21 reportability evaluation for a deviation in a basic component. The most sig-nificant of these deficiencies was the failure of the Conax Corporation to perform the required 10 CFR Part 21 evaluation for workmanship problems found on six out of twelve explosive primer chamber (EPC) assemblies that were ship-ped to Conax Buffalo from Conax Florida for final shipping to Susquehanne Nuclear Station. The workmanship problem consisted of voids and gaps around the explosive primer devices af ter a special epoxy " potting" process was per-formed. It was also revealed that subsequent undocumented repairs and accep-tance inspections were performed on the six EPC assemblies. The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.

The enclosed Notice of Violation is sent to you pursuant to the provisions of Section 206 of the Energy Reorganization Act of 1974. You are required to sub-mit to this office within 30 days from the date of this letter a written state-ment containing:    1) a description of steps that have been or will be taken to correct these items; (2) a description of steps that have been or will be taken to prevent recurrence; and (3) the dates your corrective actions and preventive measures were or will be completed. Consideration may be given to extending your response time for good cause shown.
You are also requested to submit a similar written statement for each item which appears in the enclosed Notice of Nonconformance.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter and the enclosed inspection report will be placed in the NRC's Public Document Room.
